Permission Number Requests

Please fill out the following form to request permission numbers for your Music courses.

PLEASE NOTE: Please do not request permission numbers for courses outside of the Music Department.

You can click on the + symbol in the form below to add up to seven additional rows for courses.

Permission Number Request Form

Please complete the following information to request a permission number for your Music courses. Please note that there are some courses for which the Music Office is unable to distribute permission numbers. We will notify you if that is the case.

  • Please select your program from the drop-down list.
  • Fill out this section for each course you need a permission number for. You may list up to eight courses.
