Production Form

This form needs to be completed for all events and productions in von der Mehden and the Music Building. The form must be submitted at least two weeks before the event. Requests stated on this form will not go into effect until written confirmation from the Production Manager has been received.
  • For student recitals, please list the primary performer(s) and accompanist(s) with their instrument or voice type in the following format: [First Name] [Last Name], [Instrument].
  • For any events not sponsored by UConn Music, you will need to meet with Kirk Matson, Production Manager, to discuss logistics. Please provide all of your availability to meet with Kirk prior to your event. He will reach out to schedule a time based on your availability.
  • Please indicate all equipment required for your event.
  • For any equipment selected above, please indicate the quantities needed if more than 1 (ex. 10 chairs, 2 tables, etc.).
  • Please list all repertoire to be performed in order using the following format: [Title of Larger Work (if applicable)], [Title of Piece], [Composer (birth year - death year)]. Please be sure to indicate different sets by leaving space. If you would like an intermission, please note where you would like it to occur in the program order. Please indicate the performer(s) of each piece if soloists are featured. If you would like texts and translations included as a program insert, please send them to the Production Manager as a Word document.
  • Please copy and paste your program notes as you would like them to appear in the printed program.
